Conveniently located on The Oaks of Louisiana campus, the Health Center at Live Oak offers 24/7 person-centered care to those with complex medical needs or health challenges or to those who require short-term rehabilitation. The continuous integration of all aspects of life – physical, mental, social, spiritual and emotional – makes a difference in the lives of residents at the award-winning Health Center at Live Oak.

With a long heritage of caring for older adults and the support of the health center’s parent company, Willis-Knighton, all residents have a sense of belonging and confidence that staff are tailoring care to respond to their specific needs.

Professional Staff

The Health Center at Live Oak is under the leadership of an interdisciplinary team including:

  • Licensed Administrator
  • Director and Assistant Director of Nursing Services 
  • Director of Social Services 
  • Certified Dietary Managers 
  • Certified Activity Directors

Each has one goal – to assure the best quality of life for the Health Center’s residents.

They accomplish this through: 

  • Preadmission assessment interview by the Health Center team 
  • Ongoing nursing assessment of the resident’s physical and mental status 
  • Round-the-clock coverage by licensed nurses and certified nursing assistants 
  • The services of additional full-time staff including a degreed chaplain, music therapist and other certified therapists, certified beauticians, and professional housekeeping and laundry teams.

Certification and Quality Standards

Health Center at Live Oak is a certified Medicare and Medicaid provider.

In 2007, Live Oak became a member of the Willis-Knighton Health System, giving residents access to services from a local health system that is continually recognized nationally for its quality care. The Willis-Knighton affiliation provides residents with access to a broad range of specialized health services.

Health Center at Live Oak collaborates with state and local healthcare associations.

Residents at Health Center at Live Oak are always free to choose their own doctors and hospitals.

Social Services

A wide range of social services are available to Health Center residents under the direction of a degreed social services director. The director:

  • Assesses the non-nursing needs of the resident
  • Develops a social care plan, working closely with the resident’s family, which identifies the activities that best meet these needs 
  • Holds monthly resident and family council meetings 
  • Conducts one-on-one visits with residents 
  • Obtains community services to assist the residents and/or families.
